[BUG][SPOILER] Can't finish game, crash every time loading ending (PS4 1.08)

Very frustrating, annoying bug unlike anything I've encountered in any game.

I'm level 33, finished (?) game last night, defeated Eredin and got trophy. Found Avallach in tower, and went through the dialogue sequences. Ciri goes through a portal to the blizzard, then it cuts back to the tower where the portal closes, something shatters and ice or glass falls from the sky. At this point, the game pops a...
